With same-day appointments available, getting your physical from DMC couldn’t be easier. Whether your child is getting ready for sports, school or camp, a physical is the first step in making sure they’re healthy and ready for activity.

We offer affordable options, with $35 Sports Physicals / Working Papers and $50 school entrance exams. Be sure to check with your program administrator about the necessary requirements, in order to be cleared. The requirements can vary by state and sport.

It is also important to remember that during a physical, our physicians only clear you for that specific activity or job.  If you change jobs or need a physical filled out for a different activity, you must pay an additional $35 to be seen again.

Be Prepared For Your Physical

• Bring all medical records (including shots / immunizations)
• Note any changes in behavior or mood
• Note any current medical conditions
• Bring the Physical Form (obtained from school or sport)
• Bring list of current medications

Knowing about any past illnesses, injuries or surgeries and discussing current health issues / concerns is extremely important. This information will help our physicians effectively diagnose and prevent future complications.
